New Patient Information

• Please plan to arrive 15-20 minutes before your scheduled  appointment.  To save time, download and complete our "New Patient  Paperwork" and bring it with you.
• Please bring a photo ID, insurance card, and any payment / co-payment.  If you have a high deductible insurance plan, and have not  met your deductible, you are expected to pay for the appointment prior  to seeing your therapist/nurse practitioner. Please call your insurance  company to inquire about your benefits.
• Please bring  a list of your current medications including dose, schedule, frequency. 

 Feel free to download and complete our New Patient paperwork and bring it with you upon your initial appointment.
